Job Description:
Client seeks a Healthcare Terminology Subject Matter Expert (SME) to serve as its lead technical authority for End client healthcare standards, controlled medical vocabularies, and terminology services. This individual will be the primary technical resource supporting the Medical Language Branch - including the UMLS, RxNorm, MeSH, and NIH Common Data Elements programs - as well as CMS eCQM/dQM terminology services through VSAC and the Terminology Services (TS) Repository. This is a named key personnel position in the TechnoGen proposal.
Key Responsibilities - Medical Language Branch
Maintain and implement new features in MEDIT - the editorial system client staff use to create, update, and manage MeSH vocabulary.
Maintain and enhance the MeSH Browser - the public-facing search interface for the Medical Subject Headings vocabulary.
Support MeSH production, including daily and annual MeSH updates in XML, MARC, and RDF formats.
Provide technical leadership in MeSH modernization - reducing maintenance cost, improving performance, and enriching capabilities.
Update UMLS terminology content to provide the user community with current, accurate data.
Maintain and support the UTS public browser for UMLS.
Support the RxNorm AWS cloud migration - architecture input, data validation, and release pipeline continuity.
Support RxNorm weekly and monthly data releases, ensuring accuracy and timeliness.
Implement enhancements to the RxNorm editing system and integrate innovative features using emerging technologies.
Key Responsibilities
VSAC and eCQM/dQM Terminology Services
Assess, standardize, and maintain value sets used in CMS eCQMs and dQMs, complying with CMS final rules and publication timelines.
Maintain and enhance VSAC value set authoring, expansion, review, and collaboration capabilities including the VSAC Collaboration Tool.
Support annual eCQM/dQM updates and FHIR-based measure releases, including MADiE measure development and testing.
Maintain and enhance the Terminology Services (TS) Repository - ingestion, validation, versioning, approval, and dissemination of FHIR ValueSet and CodeSystem resources.
Provide API-based and file-based mechanisms for terminology retrieval and distribution to VSAC and authorized consumers.
Support HL7 FHIR Connectathons and CMS technical assistance activities related to terminology standards.
Evaluate and advise on terminology-related standards questions from ONC and other external organizations.
Maintain ETL processes, database migrations, and standardized APIs for terminology content.
Required Qualifications
? REQUIRED Direct hands-on experience with the Unified Medical Language System (UMLS) - Metathesaurus structure, UMLS Knowledge Sources (UMLSKS), terminology sources, and related APIs or browser tools (UTS). Project names, agency names, and dates are required in the resume.
? REQUIRED Direct hands-on experience with RxNorm - concept types (IN, PIN, BN, SCD, SBD, SCDF), drug vocabulary normalization, RxNorm APIs, and the weekly/monthly release process.
? REQUIRED Direct hands-on experience with Medical Subject Headings (MeSH) - tree structures, descriptor records, qualifier records, and the MeSH XML production format.
? REQUIRED Experience with the Value Set Authority Center (VSAC) - value set authoring, CMS eCQM program value set requirements, or HL7 FHIR ValueSet resources.
? REQUIRED Experience with HL7 FHIR R4 terminology resources - ValueSet, CodeSystem, ConceptMap - and FHIR RESTful terminology APIs.
? REQUIRED 5+ years of experience in software development or technical support for healthcare terminology, biomedical informatics, or clinical data standards.
? REQUIRED SQL fluency - Oracle, PostgreSQL, or MySQL - ability to write and optimize complex queries against biomedical terminology databases.
? REQUIRED XML and XPath experience - ability to process MeSH XML, UMLS MRFILES, and FHIR XML/JSON payloads.
Strongly Preferred Qualifications
Federal agency experience at NIH, NLM, CMS, ONC, or a health IT contractor supporting these agencies.
Direct experience with NLM systems: MEDIT, MeSH Browser, UTS, RxNorm editing system, VSAC Collaboration Tool, or TS Repository.
eCQM and dQM program support - CMS Annual Update timelines, HEDIS value sets, FHIR-based measure specifications.
AWS cloud migration for data-intensive biomedical applications (RDS, S3, Lambda, EC2).
ETL tools - Kettle/Pentaho or equivalent - for biomedical terminology data processing.
Experience with NIH Common Data Elements (CDEs) repository and standards.
Advanced degree in Biomedical Informatics, Library Science, Computer Science, or related field.
HL7 FHIR Foundation Certificate or equivalent formal training.
