Role Overview
- report directly to the Manager, Care Management
- ensure optimal continuity of care for patients
- support patients transitioned in/out of services
- navigate healthcare options and Medicare coverage
- handle customer issues and complaints
- make clinical level of care determinations
- follow up and track referral and admission outcomes
- participate in Company's mandatory training
- provide education regarding Home Health, Hospice, and Palliative Care Services
- assist in admissions process
Requirements
- Associates Degree required
- BSN preferred
- compact state RN license required
- at least 3 years post-acute experience
- nursing background across post-acute care
- current CPR certification
- knowledge of home health, hospice, and palliative care services
- excellent analytical and problem-solving skills
- must read, write, and speak fluent English
