Licensed Physical Therapist – Care Navigator

Role Overview

• provide remote monitoring services to support patients through their home-exercise therapy program
• direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training
• provide ongoing feedback to the evaluating clinician on patient progress
• maintain necessary records and notes while providing remote monitoring
• assist with improving processes and functionality of the platform
• organize and manage a caseload while staying efficient

Requirements

• 2+ years of experience in musculoskeletal health is a plus
• graduated from an accredited physical therapy program
• active Compact PT license
• tech savvy, including having your own newer model laptop and smartphone
• experience in behavior change, motivational interviewing
• Health Coach Certification is a plus
• excellent communication, interpersonal, and presentation skills
• strong relationship-building and listening skills
• demonstrated patient focus
• ability to multi-task and manage various types of users
• ability to work independently with limited supervision
• flexibility and agility to work cross-functionally
• ability to influence change and bring enthusiasm
