Human Services Counselor I

Role Overview

• Establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact.
• Provide Case Management to pregnant women and infants referred to the Healthy Start Program.
• Provide face to face contact with clients either in the OB provider’s office or in the client’s residence.
• Provide Parenting Education one on one with clients.
• Provide Breastfeeding Education and Tobacco Cessation Education to individual clients.
• Document all client contact information in the web-based computer application known as the Well Family System.
• Provide referrals to community resources to supplement Healthy Start services.

Requirements

• Valid Florida Driver's License
• At least 1 year experience working with high-risk pregnant women and/or families
• Must have reliable and dependable transportation every day for home visitation purposes
• 1 to 3 years of experience working with families (preferred)
• 1 to 3 years home visiting experience (preferred)
• 1 to 3 years case management experience (preferred)
• Bilingual with fluency in English and Spanish (preferred)
• Associate's Degree or Higher in psychology, Social Work or related field (preferred)
