Coordinate residential placement for new and existing clients in partnership with residential organization staff.
Assess the service needs of adult residential clients with intellectual & developmental disabilities.
Develop individualized service plans (ISP) and crisis intervention plans with consideration of clients' specific needs, goals, and preferences.
Partner with residential support staff and leaders to identify routine needs and concerns, individuals' milestones, crisis, etc.
Connect clients to community services that promote clients' individualized goals.
Develop meaningful rapport with clients to create a safe and trustworthy relationships.
Proactively engage with clients' authorized loved ones to keep them informed, monitor service delivery and satisfaction, and promote family engagement.
Provide education to clients & their families about behavioral health illness, abuse, medication, clinical plans, and available community services.
Provide written and oral instruction to direct support staff on individualized care needs.
Develop and maintain crisis management plans and coordinate major and minor transition plans.
Deliver and/or coordinate crisis intervention services, as needed.
Observe and update client records (including physical/mental/behavioral health, medications, and side effects) in EHR systems.
Assist in developing savings and budget plans in preparation for eventual independent living.
Contribute to overall improvement of residential services under leadership of Program Manager & Program Director.
Arrange appointments/service for clients' preventative, aftercare, and follow up care.
Advocate for individuals to ensure delivery of needs, preferences, and rights.
Ensure all federal/state laws, DBHDS regulations, and company standards are met in managing clients' cases.
Requirements
Bachelor's degree in Human Service, Psychology, Social Work, Counseling, Sociology or equivalent work experience
4+ years experience managing caseloads for individuals with intellectual/developmental disabilities (ID/DD)
Valid Driver's license with acceptable driving record & reliable transportation
Effective written & verbal communication skills amongst various demographics
Conflict management and problem-solving skills
Ability to prepare case related reports including outcomes, successes, and challenges
Ability to connect with clients and assess individual needs
Experience with MS Office 265, EHR systems, and Notable (or ability to learn)
Knowledgeable of clinical assessment methods
Ability to partner with local service providers and coordinate client care
