About this role
Role Overview
Evaluates and provides family-oriented services to home health patients with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments
Conducts patient and caregiver education, discharge planning, and consultation with other professionals, clinics, or agencies
Providing initial and ongoing assessment to determine the level of functioning
Gather background data, review referrals, select and modify evaluation tools, and evaluate patients determining the level of patient’s physical and psychosocial functioning
Developing and revising the plan of care in consultation with the physician and other care-team members
Responsible for planning, evaluation, and treatment implementation of assigned caseload
Preparing clinical progress notes
Supervising therapy assistants and home health aides as appropriate
Evaluating outcomes
Planning Discharge
Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment
Responsibilities may also include the training of students from university or community colleges assigned
Requirements
Philadelphia Occupational Therapy (OT) license and CPR Certification
Demonstrate flexible and efficient time management techniques.
Strong Clinical & Interpersonal Skills
Must be able to solve problems independently and confidently by applying analytical and logical thinking.
Excellent Written & Oral Communication Skills
Prioritize workload to meet productivity standards and applicable deadlines.
Communicate with Director of Nursing on a regular basis about project status and issues.
Establish and maintain positive interpersonal relations with internal and external customers.
Maintain a positive and respectful attitude and demeanor.
Report to work on time and prepared to perform duties.
Able to use standard office equipment and software proficiently.
Adhere to all agency policies and procedures.
