Role Overview
- Monitor patient data and assess clinical information
- Provide timely interventions to support patient safety and care outcomes
- Collaborate closely with interdisciplinary care teams to reinforce treatment plans
- Conduct virtual follow-up appointments to guide patients
- Document clinical interactions thoroughly and prepare detailed care summaries
Requirements
- Multi-state RN Compact State Licensure and a single state RN license in NY.
- Bilingual in English and Spanish is required
- Associate Degree in Nursing Science.
- 5+ years of recent clinical experience as an RN in an ICU/ER setting is required.
- Prior experience supporting patients in a chronic care management program is a plus.
- Skilled in nursing processes.
- Excellent clinical acumen.
- EMR experience, preferably in Athena and EPIC.
- Patient assessment competency.
