Role Overview
Provide comprehensive Social Care Navigator services, including screening & navigation services to eligible Medicaid Members via phone, in-person, & virtually
Document within the Unite Us electronic health record
Develop and maintain an in-depth knowledge and understanding of the range of services (including eligibility criteria) available in the Social Care Network and existing local social services infrastructure
Participate in all required SCN trainings and meetings
Develop care plans for clients based on eligibility for services and identification of needs
Communicate with community agencies and service providers
Follow established agency policies and maintain professional standards
Perform other duties as assigned
Requirements
Associate’s degree in a relevant field or equivalent
Bilingual capability (Spanish/English)
Basic computer knowledge: typing, communication tools, scheduling tools and web browsers
Proficiency in the Microsoft Office suite and database management systems
Fluency in English (Reading, Writing, Speaking)
Valid driver’s license
Must obtain and maintain Defensive Driver Certification within 30 days of hire
Must be and maintain clearance by applicable regulatory clearances as required
Legally eligible to work in the United States without sponsorship
