Bilingual Behavioral Health Care Manager

Role Overview

• Provide high quality and effective care management to Titanium Healthcare members
• Support ECM members with behavioral health conditions, focusing on SMI and/or SUD needs
• Case manage members with behavioral health needs, utilizing brief behavioral health therapeutic interventions
• Conduct comprehensive, holistic assessments both telephonically and in person
• Engage and collaborate with staff outside the BH department to provide integrated care
• Coordinate with internal and external health partners to support member comprehensive care needs
• Advocate for members to receive high-quality care within IEHP’s network
• Participate in care team meetings and manage transitions of care

Requirements

• Master’s degree in social work or counseling
• Minimum 1+ years of experience in a social service setting, such as case management or community health
• Current and valid Driver’s License with a clean driving record
• Reliable transportation and proof of auto insurance
• Current BLS certification from the American Heart Association upon start date
• Distraction-free home workspace with a secure internet connection
• EHR/EPM experience; eClinicalWorks (eCW) a plus
