Licensed Physical Therapist – Care Navigator

Role Overview

• Provide remote monitoring services for patients
• Direct and assist users in therapeutic exercise and functional training
• Provide ongoing feedback to evaluating clinician on patient progress
• Maintain necessary records while providing remote monitoring
• Assist with improving processes and functionality of the platform
• Organize and manage a caseload

Requirements

• 2+ years of experience in musculoskeletal health is a plus
• Graduated from an accredited physical therapy program
• Active New York PT license
• Tech Savvy
• thrive in a multi-platform environment
• Experience in behavioral modification, Health Coach Certification is a plus
• Excellent communication skills
• Demonstrated patient focus; a genuine passion for the user and their experience
• Ability to multitask and manage various types of users
• Ability to work independently with limited supervision
• The flexibility and agility to work cross-functionally in a fast-paced environment
• Passion for health and wellness
