Role Overview
- Provides case management/therapy for patients and families to enhance their psychosocial functioning.
- Ensures safety, confidentiality, regulatory compliance, and adherence to hospital and infection control standards.
- Coordinates interdisciplinary care, advocates for patients and families, and oversees safe, comprehensive discharge planning.
- Provides behavioral health case management, assessments, therapy, crisis intervention, and ongoing treatment planning with accurate documentation.
- Communicates effectively with patients, families, and community partners while maintaining strong clinical, organizational, and technology skills.
Requirements
- Master's degree required in Social Work, Counseling or another human-service related field
- Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) in Missouri required.
- Minimum 2-3 years Behavioral Health experience preferred.
- Co-worker must be able to communicate orally, in writing and demonstrate proficient computer skills.
- Co-worker must be able to listen and to understand and explain complex situations.
