Role Overview
- Provide monitoring, follow-up, and clinical care management to dually-eligible enrollees
- Collaborate with healthcare providers and community resources for seamless communication
- Engage in care plan development and provide routine updates
- Advocate for enrollees' needs within the healthcare system
- Conduct required assessments including Comprehensive Assessment and Crisis Assessments
Requirements
- Associates of Science (A.S) degree in nursing or Master's degree in social work/mental health counseling
- Current unrestricted clinical license in Massachusetts as RN/LCSW/LISCW/LMHC
- Three years of experience as a Registered Nurse/BH Clinician or one year with additional two years experience as LPN/CHW/MA
- Intermediate proficiency with Microsoft Office
- Understanding of Medicare and Medicaid programs
- Strong decision making and problem-solving skills
