Social Work Case Manager

Role Overview

• Ensures effective psychosocial intervention positively impacting patients' ability to manage chronic illness
• Utilizes community, government, and/or client resources to address participants' limitations
• Manages behavioral and psychosocial needs
• Assists members to effectively utilize available resources for personal health needs
• Evaluates members' ability to manage self and locate alternative resources
• Provides guidance seeking alternative solutions to specific problems
• Evaluates strengths for health self-management and develops strategies for healthcare needs

Requirements

• Requires MS (at a minimum) in Social Work
• Minimum of 3 years of experience in case management in a health care environment
• Current unrestricted LMSW or LCSW (or equivalent) license in applicable state(s) required
• Bilingual (Spanish) or multi-language skills preferred
