Provides video consultation/triage to patient, family, and significant others according to the Home Health Virtual Program plan of care
Provides ongoing care via video according to the established plan of care
Determines priorities of patient care based on essential patient needs and available resources
Coordinates patient interventions such as physician contacts, consultation with patient's home health care team, facilitation of appropriate visit strings, changes in physician orders, and care plan goals, in cooperation with the patient’s physician, as appropriate
Assesses patient, family and/or caregiver’s training needs and provides teaching/counseling video consultation/triage
Collects and maintains clinical and patient records according to agency, state, and federal guidelines
Requirements
Occupational Therapy license issued by the state in which the teammate practices
Bachelor of Science, Masters, or Doctorate Degree in Occupational Therapy
5 years of experience in combination of rehab, home care and triage settings
Must possess strong communication skills
Advanced computer skills using Microsoft Office applications
