Case Manager, Registered Nurse

Role Overview

• Conduct telephonic (and occasional in-person) member assessments
• Develop and manage personalized care plans to improve health outcomes
• Identify and address clinical, social, and functional barriers to care
• Coordinate resources, referrals, and transitions across care programs
• Collaborate with interdisciplinary teams to support complex cases
• Engage members through effective communication to drive positive results

Requirements

• Active, unrestricted multistate RN license (or willingness to obtain additional licenses—company paid)
• 3+ years of clinical RN experience
• Comfort navigating multiple systems (Microsoft Office, web-based tools)
• Strong critical thinking, communication, and member engagement skills
• Associate’s Degree in Nursing
