Role Overview
- Deliver comprehensive ambulatory care management services to identified patients
- Conduct care management screenings, assessments, and evaluations
- Develop and implement patient-centered care plans
- Collaborate with patients, families, healthcare providers, and community resources
- Monitor and evaluate the effectiveness of care plans
Requirements
- Current Registered Nurse (RN) license in state of practice
- Bachelor of Science in Nursing (BSN) from an accredited institution (degree verification required)
- Clinical nursing experience in chronic disease management
- Proficiency in basic computer skills and Microsoft Office
- Current BLS certification if required to conduct home or community visits
- Current driver’s license and reliable transportation
