Physical Therapist, Pelvic Health Float/Virtual Outpatient

Role Overview

• Interprets physician referrals and completes evaluations utilizing appropriate methods
• Establishes a plan of care and implements individual treatment based on evaluation results
• Monitors and evaluates outcomes and adjusts individualized treatment plan
• Plans for and completes patient discharge including providing written home programs
• Completes all required documentation following department policy
• Responsible for cleaning up after patient treatment and maintaining area neat and orderly
• Participates in performance improvement activities designed to identify and resolve problems
• Pursues professional growth through continuing education courses and participates in the evaluation process of co-workers

Requirements

• Bachelor's Degree in Physical Therapy
• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months
• Physical Therapist license issued by the state in which the team member practices (Wisconsin or Illinois)
• Intermediate computer skills
• Pediatric experience is preferred
