Virtually assess patients referred by our physician members with a range of physical and functional concerns.
Provide self-management advice, education, and referral/navigation support to CWC PCN contracted physiotherapy clinics and/or community programs.
Gather and analyze information about the issue/concern from referral forms and through phone consultation with the patient.
Interpret assessment findings and other relevant information.
Work with the team’s program coordinator to book patients for screening and assessment.
Interview patients to obtain relevant information about health conditions and personal and environmental factors.
Determine the patient’s expectations and their relevance to physiotherapy.
Develop physiotherapy diagnoses and plans for treatment.
Refer patients to appropriate services based on assessment and eligibility.
Provide self-management education to patients, as needed.
Maintain accurate patient records in an Electronic Medical Record (EMR).
Communicate referral outcomes and provide consult letters to referring physicians.
Requirements
A bachelor’s or master’s degree in physiotherapy from an accredited university program is required.
A minimum of five years’ clinical experience in a variety of settings (e.g., acute, community).
The ability to manage a wide variety of clinical situations with varying levels of acuity, complexity, and diverse populations.
A minimum of two years’ experience with the following is an asset: experience in the delivery of virtual or telerehabilitation interventions, experience with trauma informed care.
Active registration with the College of Physiotherapists of Alberta is required.
Certifications in diagnostic imaging and vestibular and concussion management are an asset.
Proficiency in the Microsoft Office Suite (Word, Excel, Outlook, PowerPoint, Teams) and EMR systems (MedAccess, CHR).
A clear Criminal Background check with Vulnerable Sector check is required.
A current Health Care Provider CPR certificate (BLS Provider from the Heart & Stroke Foundation) is required.
