Nurse Practitioner – Telehealth

Role Overview

• Conduct virtual patient assessments and develop personalized care plans.
• Diagnose and treat acute and chronic health conditions via telehealth platforms.
• Educate patients on health management and preventive care strategies.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care.
• Document patient interactions and care plans in electronic health records (EHR).
• Stay updated on telehealth regulations and best practices to ensure compliance.
• Provide follow-up care and support to patients as needed.
• Participate in quality improvement initiatives to enhance patient outcomes.

Requirements

• Master's degree in Nursing with a focus on Nurse Practitioner training.
• Current Nurse Practitioner license in Georgia and board certification.
• Minimum of 2 years of clinical experience, preferably in telehealth.
• Strong communication skills and a compassionate demeanor.
• Proficiency in using telehealth technology and electronic health records.
• Ability to work independently and manage time effectively in a remote setting.
• Commitment to providing patient-centered care and continuous learning.
• Knowledge of telehealth regulations and best practices is a plus.
