Role Overview
- assess and evaluate member's needs
- guide members/families toward appropriate resources
- administer ongoing long-term services and support assessments
- contact members for evaluation of resources and plan of care
- collaborate with care team to ensure cohesive service delivery
- assist members in maintaining Medicaid eligibility
Requirements
- Bachelor's degree in health and human services field
- 2 or more years of related experience
- valid state driver's license
- personal vehicle liability insurance
- separate room with a locked door for home office
- knowledge of community health and social service agencies
