Role Overview
- Develop/maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management
- conduct facility visits to assist program clinical team
- ensure effective communication and collaboration with program staff
- participate in weekly program business development meetings
- assist program in processing of physician orders
Requirements
- RN/LPN/LVN license
- Bachelor's of Science in Nursing preferred
- 3 years clinical experience preferred
- strong knowledge of governmental regulations
- Medicare eligibility requirements understanding
- coordination of necessary resources
