Case Manager, Registered Nurse

Role Overview

• Assess, plan, and coordinate care for members via phone
• Develop proactive care strategies for health outcomes
• Use clinical data and assessments to connect members to programs
• Address complex medical and social factors impacting health
• Engage members through effective communication and clinical insight
• Ensure compliance with regulatory and organizational guidelines

Requirements

• Active, unrestricted RN license
• 3+ years of RN experience (acute care, ICU, case management, discharge planning, or similar)
• Willingness to obtain additional state licenses (company paid)
• Strong communication and multitasking skills in a tech-enabled environment
• Ability to work independently in a distraction-free home office with reliable internet
• Proficiency with Microsoft Office and multi-system navigation
