Role Overview
- Contribute to administration of care management
- Provide non-clinical support to the assessment and evaluation of members' needs
- Guide members/families toward and facilitate interaction with resources
- Conduct in-person assessment(s) to understand member care needs
- Assist enrollees with scheduling physical health and behavioral health (BH) office visits
- Be the member advocate with providers, community resources, schools, and other entities
- Promote and monitor member adherence with their care plan
- Support members through culturally appropriate health education and health coaching
- Attend Humana community events to connect with members
- Build and maintain relationships with providers and community resources
Requirements
- Must reside in Illinois
- Minimum two (2) years prior experience working with community resources, community health agencies/social service agencies
- Valid driver's license, car insurance, and access to an automobile
- Strong understanding and respect of all cultures and demographic diversity
- Must be able to work a 40-hour work week, Monday through Friday 8:00 AM to 5:00 PM
- Bachelor's degree in social work or related health field (preferred)
- Community Health Worker training and/or certificate (preferred)
- Familiarity with state Medicaid program guidelines (preferred)
- Experience engaging with Medicaid enrollees, including those with physical health and behavioral health needs and varied health literacy (preferred)
