Interpret physician referrals and complete evaluations utilizing appropriate methods
Establish a plan of care and implement individual treatment based on evaluation results
Monitor and evaluate outcomes and adjust treatment plans as necessary
Plan for and complete patient discharge including providing written home programs
Document patient care and maintain productivity standards
Participate in performance improvement activities and orient new staff as required
Requirements
Doctorate degree in Physical Therapy from an accredited school
Licensed Physical Therapist in the State upon which you work
Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)
Intermediate computer skills
Excellent communication, interpersonal, and organization skills
Must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served
