Position Summary:
Trillium Health Partners (THP) operates a 39 bed post-acute medically Complex Continuing Care (CCC) unit at our Queensway Health Centre (QHC) location and is recruiting an individual who is highly skilled, agile and adaptable with advanced critical thinking and interprofessional skills. As a member of the CCC, QHC Interprofessional team you will have the opportunity to provide care to the medically complex continuing care patient population.
