Licensed Practical Nurse - Lafayette PACE

The Licensed Practical Nurse provides basic therapeutic nursing care according to established standards of practice under the direction and supervision of the RN.  Provides care to those patients and caregivers whose care requirements are routine and standardized with predictable outcomes.  Assists the RN and healthcare team to achieve desired patient outcomes.  Relies on education, training and established guidelines to complete responsibilities.  Initially requires instruction and guidance to accomplish job which should develop into working under general supervision. 

1. Participant Care
   1. Direct participant care is provided utilizing the nursing process under the direction and supervision of the healthcare team at all times.
   2. Diagnostic test results are reviewed and abnormal results and changes in participant condition are reported to the RN, physician and appropriate healthcare team members immediately.
   3. Prescribed medications, therapies and treatments are administered in accordance with practical nursing standards at all times.
   4. Participant medical records, notes and physician orders are accurately reviewed and maintained; All participant care activities are accurately documented in accordance with documentation standards at all times.
   5. Components of the plan of care within the LPN scope of practice and competency are implemented to promote desired outcomes at all times.
   6. Assistance is provided with discharge planning and carrying out the participant's teaching plan under the direction of the healthcare team at all times.
   7. Completes all medication orders in the PACE Care in an orderly and consistent manner.
   8. Assists with accurate documentation in the medication realm and assists with proper refill of medication processes.
2. Coordination of Care
   1. The execution of participant's plan of care is effectively collaborated with the supervising Healthcare Team at all times.
   2. Physicians, RN's and health care team members are effectively assisted with data collection, assessment, planning, intervention and evaluation of participant and families care to achieve desired outcomes.
   3. As an advocate for the participant, supports the participant's psychological, ethical, spiritual and cultural needs by collaborating with the RN and healthcare team.
   4. Healthcare team members are sought out to serve as a resource to increase practice knowledge.
   5. Active participation on unit teams, work groups and/ or committees is provided as requested; anticipates needs and voluntarily assists others.
   6. Outcomes of the team/committees' work are supported and put into practice; keeps informed of Unit initiatives.
3. Quality
   1. Provides accurate input of all medications into PACE Care to help ensure 0% medication errors as required by the FMOLHS dashboard.
   2. Knowledge of unit based quality improvement initiatives is demonstrated at all times.
   3. Active participation in Quality improvement initiatives is provided by utilizing standards, guidelines, and pathways for care delivery.
   4. Provides knowledge of problems with medication delivery and processes with the Participant, and/or caregivers to the Interdisciplinary team, for solutions to be prepared.

Experience - Must have 1 year experience working with the frail elderly population 1 year experience working as LPN

Education - Graduate of Accredited LPN program High School

Training - Graduate of Accredited LPN program

Special Skills - Proficient in English, verbal and written communication skills and computer skills
