Social Worker - BSW PRN Premium

The Medical Mgmt Social Worker 1 provides patient and family assessments in order to design effective treatment programs and follows the patient and family through the continuum of services. The Medical Mgmt Social Worker 1 provides supportive counseling to patients and families, assesses patient discharge needs and coordinates discharge services. Relies on education, experience, professional training and judgment to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision.

   4. Community resources are effectively worked with/coordinated and keeps current on new resources in the community as evidenced by documentation.
