The Discharge Medication Program Representative is responsible for cultivating potential program participants while providing a delivery service for patients discharging from inpatient services. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. INTEGRIS Health is an Equal Opportunity/Affirmative Action Employer.
The Discharge Medication Program Representative responsibilities include, but are not limited to, the following:
- Develops and cultivates relationships with targeted referral sources.
- Obtains referral and patient's insurance information.
- Educates patients about the program by meeting with them individually, provides informational program materials and offers a consult from a pharmacist.
- Maintains documentation of all HIPAA forms and delivery tickets
- Works with hospital staff to refer indigent patients to INTEGRIS Patient Assistance Program or other applicable assistance programs.
- Handles delivery of medication to discharging patients room and follows up with staff once complete.
- Resolves rejections from pharmacy payors on medications.
Reports to the Retail Pharmacy Manager.

- Pharmacy Technician License issued by Oklahoma State Board of Pharmacy or National Pharmacy Technician Certification.
- Associates degree preferred.
- 2 years healthcare or retail pharmacy experience.
- Knowledge of Cover My Meds, foundation assistance programs, CRX, Medicare B & D, how to fill and check pharmacy scripts and co-pay and manufacture coupon assistance preferred
- Strong communication skills.
- Must be able to communicate effectively in English (verbal/written).
- Intermediate computer skills to include knowledge of Microsoft Office and other applicable computer systems.
