Clearpath is a company focused on enhancing patient care and driving interoperability workflows. They are seeking an accomplished Principal Software Engineer to lead the architecture, development, and delivery of enterprise-grade healthcare technology systems, with significant technical leadership responsibility across various healthcare domains.
Responsibilities:
- Architect and lead development of PHP/Laravel-based backend services, APIs, and microservices supporting clinical and administrative healthcare workflows
- Own cloud infrastructure on AWS, including ECS/Fargate deployments, RDS, S3, CloudFront, API-Gateway, IAM, and related services, with a focus on cost optimization, security, and availability
- Maintain CI/CD pipelines using a combination of Bitbucket Pipelines and AWS CodeBuild, or equivalent, ensuring automated testing, code quality gates, and reliable deployment workflows
- Maintain containerization efforts using Docker and orchestration via ECS, enabling reproducible, portable deployments across environments
- Collaborate with client clinical informatics, HIM, and internal product teams to translate complex healthcare requirements into maintainable technical specifications and system designs
- Mentor and provide technical guidance to mid-level and senior engineers, setting engineering standards and conducting code reviews
- Participate in architecture review boards, contribute to technology roadmap planning, and evaluate emerging technologies for platform fit
- Ensure all systems adhere to HIPAA, SOC2 Type II, and applicable regulatory requirements, with particular attention to PHI data handling, audit logging, and access controls
Requirements:
- 10+ years of professional software engineering experience, with a minimum of 5 years focused on PHP and the Laravel framework in production environments
- Demonstrated expertise in building RESTful and GraphQL APIs, service-oriented architectures, and multi-tenant SaaS platforms using Laravel
- Hands-on experience with DICOM protocols and DICOMweb standards, including practical work with imaging modalities (CT, MRI, X-Ray, Ultrasound) and PACS/VNA systems
- Working knowledge of HL7 v2 messaging (ADT, ORM, ORU message types) and FHIR R4 resource modeling for healthcare interoperability
- Proficiency with AWS services relevant to healthcare workloads: EC2, ECS, Fargate, Lambda, RDS/Aurora, S3, SQS, SNS, CloudWatch, Secrets Manager, and WAF
- Strong containerization experience with Docker and deployment automation using Docker Compose, ECS task definitions, or Kubernetes manifests
- Proven CI/CD pipeline development and maintenance experience with modern tooling (Bitbucket Pipelines, GitHub Actions, AWS CodePipeline/CodeBuild, Jenkins, or equivalent)
- Solid understanding of relational database design and query optimization (PostgreSQL) as well as experience with NoSQL data stores (DynamoDB / Redis / Valkey) where appropriate
- Commitment to secure software development practices, including OWASP adherence, secrets management, and zero-trust network design
- Experience with Health Information Management (HIM) workflows including release of information
- Familiarity with DICOM and DICOMweb
- Experience integrating with or building components of radiology information systems (RIS) and/or PACS
- Exposure to HTJ2K, JPEG 2000, or other lossless/near-lossless medical image compression and transcoding pipelines
- AWS certifications such as Solutions Architect (Associate or Professional) or DevOps Engineer are a strong plus
- Experience with OpenTofu, Terraform, or AWS CDK for infrastructure-as-code in healthcare or regulated cloud environments
- Familiarity with OHIF Viewer, dcm4chee, Orthanc, or other open-source medical imaging platforms
- Background in Go, C#, and/or C++ as a secondary language for performance-sensitive imaging workloads
