Position Details:
- Main Office location: Londonderry, NH
- Service Area: Exeter, NH and Surrounding Areas
- Job Type: Full Time
- Schedule: Monday - Friday
- Salary Range: $75,000.00 - $95,000.00
Responsibilities:
- Provide psychosocial assessments and emotional support to hospice patients and their families throughout the end-of-life journey.
- Build trusting relationships with patients, families, and caregivers while providing counseling and guidance.
- Coordinate with the interdisciplinary hospice team, including nurses, physicians, and other clinicians, to support patient care plans.
- Participate in Interdisciplinary Group (IDG) meetings and daily operational briefings to ensure coordinated patient care.
- Connect patients and families with community resources and support services.
- Maintain accurate and timely documentation in the electronic medical record system.
- Communicate patient status updates and psychosocial needs to the clinical team.
- Support families through grief, adjustment, and end-of-life planning.
- Follow all patient confidentiality, safety, and infection control protocols.
- Participate in case reviews, continuing education, and training programs as required.
- Participate in on-call responsibilities as assigned.
Qualifications:
- Current LICSW license in the state of practice is preferred. We are able to consider LCSW licenses as well.
- Master’s degree in Social Work from an accredited program.
- Two or more years of healthcare or hospice experience preferred.
- Current CPR certification with in-person skills verification.
- Valid driver’s license, reliable transportation, and required insurance.
- Strong interpersonal, communication, organizational, and problem-solving skills.
- Ability to collaborate effectively within an interdisciplinary care team.
- Ability to travel to patient homes and community settings as needed.
- Experience with electronic medical records systems preferred (Homecare Homebase experience a plus).
