
JOB SUMMARY
Under the supervision of the Clinical Director, the licensed or licensed eligible Clinician at the Old Colony YMCA's STRIVE Program is responsible for ensuring that proper clinical and case management services as well as community reintegration planning are provided to youth participants referred by the Department of Youth Services (DYS). This role will have a large outreach component as the Clinician will act as a liaison between participants and community resources as they begin their process of transitioning to a more independent status within this community.
Typical Job Responsibilities Include:
· Ensure that an intake screening (suicide/safety assessment) is completed for each new participant in the program.
· Provide weekly individual therapy to participants and complete required Department of Youth Service (DYS) clinical progress notes.
· Provide group therapy several times a week.
· Provide family therapy as needed.
· Develop an individual service plan for each participant and create monthly treatment plan reviews with individualized goals and progress reports.
· Schedule and facilitate participant treatment meetings (monthly) to review progress. Act as a liaison as needed between youth participant, family, and DYS.
· Assess and monitor all participants on suicide watch in conjunction with the Clinical Director.
· Research and develop connections to community resources including, but not limited to: employment opportunities and workforce training agencies in the community, educational programs, volunteer centers, and recreational activities.
· Support Youth Care Advocates (YCAs) as mentors to the participants of STRIVE.
· Provide training opportunities for YCAs in the areas of mental health, life skills, and asset coaching.
· Ensure that all program records and participant files are accurate and up-to-date, including assisting with audit preparation, weekly filing, requesting records from DYS.
· Attend all trainings as scheduled.
· Report any suspected child abuse to the Clinical Director.
· Assist with the volunteer and internship programs as necessary.
· Adhere to all fiscal policies of the Old Colony YMCA.
· Ensure DYS strategic plan is implemented as indicated.
Education & Experience Requirements:
- Master’s Degree in Social Work, Counseling or a related field.
