Position Summary
The Hospice Dietitian provides specialized nutritional care to patients with life-limiting illnesses, focusing on comfort, symptom management, and quality of life. Collaborates with the Interdisciplinary Team (IDT) to align care with patient goals and hospice philosophy. Ensures all care meets CMS Conditions of Participation (CoPs) and agency standards.
Essential Duties and Responsibilities
- Conduct comprehensive nutritional assessments
- Develop individualized care plans focused on comfort
- Address anorexia, cachexia, dysphagia, hydration concerns
- Participate in weekly IDT meetings
- Educate patients and families on end-of-life nutrition
- Document in EMR (Consolo, WellSky, Careficient)
- Maintain compliance with CMS regulations
- Participate in QAPI and performance improvement
Requirements
Qualifications
- Registered Dietitian (RD/RDN)
- Licensed Dietitian (state-specific)
- Minimum 1 year clinical experience
- Hospice experience preferred
