What You’ll Do
• Provide direct patient care, including assessments and care plan development
• Manage a caseload and coordinate care across the clinical team
• Administer medications and treatments per physician orders
• Monitor patient progress and adjust care plans as needed
• Educate patients and caregivers on care and self-management
• Communicate with physicians and care teams to ensure continuity of care
• Supervise and support HHAs and LPNs
• Complete accurate and timely documentation (OASIS, visit notes, orders)
• Ensure compliance with policies, regulations, and HIPAA standards
What You Bring
• Active RN license
• Graduate of an accredited nursing program (BSN preferred)
• Home health or related behavioral health clinical experience
• Strong communication, organization, and critical thinking skills
• Current CPR certification
• Valid driver’s license and reliable transportation
Physical Demands & Environment
• Mobility: Frequent standing, walking, sitting, climbing, balancing, stooping, and kneeling.
• Dexterity: Continuous use of hands and fingers for fine motor tasks and equipment operation.
• Lifting/Transfer: Occasional lifting or pushing of loads (both under and over 25 lbs); includes occasional patient transfers.
• Hazard Exposure: Frequent exposure to bloodborne/airborne pathogens and infectious materials.
