Our Clinical Care Managers lead a team of RNs and LPNs to deliver high-quality home health care services. This role ensures care is provided in accordance with physician orders, regulatory requirements, and company policies, while supporting team performance and patient outcomes.
What You’ll Do
• Lead and support a high-performing clinical team
• Coordinate patient care to ensure quality and continuity
• Monitor daily reports, on-call activity, and clinical documentation (including OASIS)
• Partner with leadership to track KPIs and improve performance
• Support onboarding, training, and development of clinicians
• Assign visits and manage caseloads in collaboration with operations
• Conduct case load reviews and field supervisions
• Ensure compliance with regulations, care plans, and documentation standards
• Address patient concerns and follow up on field incidents
What You Have
• Active RN license
• Bachelor’s degree with 1+ year experience or Associate’s degree with 2+ years experience
• 2+ years of clinical management experience
• Strong knowledge of home health practices and compliance
• Valid driver’s license and reliable transportation
• Current CPR certification
Preferred Qualifications
• Supervisory experience
• Home health experience
• OASIS certification
