Nexus at Elmwood Park is seeking an experienced and motivated Director of Rehabilitation to lead our therapy department and oversee the delivery of high-quality rehabilitation services in our skilled nursing and post-acute care setting. Located at 7733 West Grand Avenue in Elmwood Park, Illinois, our facility is dedicated to providing exceptional care and outcomes for our residents.
Position Summary
The Director of Rehabilitation is responsible for the overall management, coordination, and supervision of rehabilitation services, including physical therapy, occupational therapy, and speech-language pathology. The DOR will ensure regulatory compliance, drive clinical excellence, manage staffing and budgets, and collaborate with interdisciplinary teams to maximize resident outcomes.
Essential Responsibilities
- Direct and oversee all rehabilitation services and therapy personnel.
- Ensure compliance with federal, state, and facility regulations.
- Monitor therapy utilization, productivity, documentation, and outcomes.
- Develop and implement programs to enhance resident functional independence.
- Coordinate interdisciplinary care planning and discharge planning.
- Recruit, mentor, and evaluate therapy staff.
- Maintain positive relationships with residents, families, physicians, and referral sources.
- Ensure timely and accurate completion of clinical documentation and billing requirements.
- Participate in quality assurance and performance improvement initiatives.

Requirements
Qualifications
Required:
- Current, active Illinois license in one of the following disciplines:
- Physical Therapist (PT)
- Physical Therapist Assistant (PTA)
- Occupational Therapist (OT/OTR)
- Certified Occupational Therapy Assistant (COTA)
- Speech-Language Pathologist (SLP/CCC-SLP)
- Bachelor's, Master's, or Doctorate degree from an accredited therapy program relevant to licensure.
- Minimum of three (3) years of rehabilitation experience in a skilled nursing facility, long-term care, post-acute care, or rehabilitation setting.
- Minimum of one (1) year of supervisory or management experience.
- Strong knowledge of Medicare, Medicaid, PDPM, regulatory compliance, and rehabilitation operations.
- Excellent leadership, communication, and organizational skills.
Preferred:
- Previous Director of Rehabilitation or Therapy Management experience.
- Experience with electronic medical records and rehabilitation management systems.
