
Position Purpose
Trinity Health at Home is seeking a compassionate and skilled Medical Social Worker (PRN) to provide consultation, assessment, and support services to homebound patients and their families. In this role, you will help address complex social and emotional challenges that may impact a patient’s ability to manage their medical condition, promoting improved health outcomes, independence, and quality of life.
What You Will Do
Conduct comprehensive psychosocial assessments of patients and caregivers.
Develop and implement individualized, person-centered plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
Identify social, emotional, environmental, and financial barriers to care and connect patients with appropriate community resources.
Provide counseling and support to patients and families experiencing complex social or emotional distress.
Assist patients and caregivers in understanding and following medical recommendations.
Collaborate with physicians and healthcare professionals to address factors impacting patient health and well-being.
Continually evaluate patient needs and modify care plans as appropriate.
Participate in care conferences, quality improvement initiatives, and interdisciplinary team meetings.

Minimum Qualifications
Master’s Degree in Social Work from a Council on Social Work Education (CSWE) accredited program.
Current Social Work license as required by state regulations.
Current CPR certification.
Minimum of two (2) years of social work experience in a healthcare setting.
Ability to provide services to acutely and chronically ill patients.
Strong communication, assessment, and critical thinking skills.
Commitment to patient confidentiality and professional ethics.
