
The Nurse Practitioner/Physician Assistant will report to the Medical Director and/or Supervising Physician and responsible for overseeing the progress of a specific caseload of endocrinology patients.
Provides inpatient to outpatient transition care for patients with Diabetes in our Ambulatory Endocrine practice located at Chapman Street in Providence, RI - Brown University Health and 375 Wampanoag Trail in East Providence, RI.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Provide consultative diabetes care to hospitalized patients at Rhode Island Hospital. Availability to work 1:4 weekends. No evening on-call activity.
Provide inpatient to outpatient transition care for patients with Diabetes in our Ambulatory Endocrine practice located at Chapman Street in Providence, RI - Brown University Health.
Work alongside and with providers, staff and care team members to coordinate care
Participate in medical education, quality improvement and research initiatives as determined by members of the division.
Responsible for ensuring good communication with patient and consulting providers.
Work with and be responsible to the Division Director, Endocrinology.
REQUIREMENTS:
Board Eligible/Board Certified
Licensed in Rhode Island
Graduate of an accredited NP/PA program
Pay Range:
