Role Overview
Deliver Medical Nutrition Therapy (MNT) to patients with chronic conditions via video, phone, email, and portal
Conduct comprehensive nutrition assessments and develop individualized care plans
Provide counseling on dietary modifications
Use Medically Tailored Meals as educational tools to support patient learning and behavior change
Leverage Remote Patient Monitoring data and portal feedback to adjust treatment plans and optimize patient outcomes
Monitor patient progress through regular follow-ups, portal engagement, and RPM data
Maintain accurate, timely documentation in the EMR system following OnPoint Nutrition protocols and regulatory requirements
Ensure compliance with HIPAA, state licensure requirements, and professional practice standards
Collaborate with peers through case consultations and team learning
Requirements
Minimum of bachelor’s degree in nutrition, dietetics, or a related field required
Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential from CDR required
Active state licensure in AL, FL, NC, NJ, NY, PA, or VA, or the ability to obtain multi-state licensure, is required
2+ years of clinical nutrition experience required
Experience with EMR systems required
Strong interpersonal and communication skills; comfort with telehealth technology required
Experience in chronic disease management and value-based care across underserved populations, including individuals with disabilities, uncontrolled diabetes, post-acute care needs, uncontrolled hypertension, and other complex conditions, preferred
Certification in obesity and weight management (CSOWM) or diabetes care and education specialist (CDCES) preferred
Familiarity with Remote Patient Monitoring tools preferred
Patient-centered approach, with the ability to demonstrate empathy and cultural sensitivity
Strong attention to detail and commitment to accurate documentation
Open to learning and receptive to receiving feedback
Ability to manage time effectively and build clinical efficiency
