About this role
Role Overview
Serve as an advocate for members, ensuring they receive needed care
Develop personalized care plans tailored to needs
Identify and catalog available community or national resources
Connect members to community services and follow up
Assist low-income members in applying for government benefits
Work with family members to understand the member's needs
Conduct calls to members
Coordinate closely with other teams
Form strong relationships with local community organizations
Requirements
Minimum of 2 years doing case management or care coordination
Ability to support either Mountain or Pacific time zones
Health insurance (preferably Medicare Advantage) and/or Healthcare experience
Strong verbal and written communication skills in English
Ability to multi-task: listening, talking, evaluating and writing notes simultaneously
Ability to understand and meet performance metrics
Proficient in technology and comfortable with new technologies
Proven success in building relationships
Great team member with a can-do attitude; self-reflective
Ability to work in a start-up, fast-paced environment
