About this role
Role Overview
Responsible for scheduling and supervising in-home care workers and clients in a geographic area.
Conducting in-home visits at clients' homes to monitor and report on quality of services being provided by Home Care Aides.
Determining whether existing care plans are adequate to meet client needs.
Creates work schedules by entering schedules into the system.
Manages changes to client schedules due to client request, illness, vacation or leaves of absence.
Provides alternate coverage to ensure the clients care plan is followed and client services are not interrupted.
Contacts care providers and clients to provide service updates.
Monitors clients condition and performance of duties by Home Care Aides.
Maintains all company and Department on Aging rules, regulations, and standards.
Prepares and completes accurate evaluation reports and case notes as needed.
Attends weekly branch meetings.
Requirements
Minimum of 3 years experience working in the field of senior services or a related social service field.
Excellent oral and written communication skills.
Strong interpersonal skills.
Ability to prioritize and handle high call volumes.
Client-focused and customer service-driven.
Proficient in Microsoft Word, Excel, and Outlook.
Must have reliable transportation, valid drivers license and insurance for travel within the branch service area
