Bilingual Analyst, Case Management – Field

Role Overview

• Assessing, planning, implementing, and coordinating all case management activities with members
• Evaluating the medical needs of the member to facilitate the member's overall wellness
• Effectively managing a caseload that includes supportive and medically complex members
• Developing a proactive course of action to address issues presented to enhance short and long-term outcomes
• Enhancing a member's overall wellness through integration
• Determining appropriate services and supports based on member's health needs

Requirements

• Bilingual, English, and Spanish.
• Willing and able to travel 50-75% of their time to meet members face to face in Miami Dade county and surrounding areas in FL.
• Must reside in Hialeah Florida, applicable zip codes 33010, 33012, 33014 or 33015.
• 1+ year of experience in behavioral health or a long-term care setting.
• Computer Proficiency in Microsoft Word, Excel, Outlook required.
