About this role
Role Overview
Develops a proactive plan of care to address identified issues to enhance member outcomes
Uses clinical tools and information/data review to evaluate member's needs and benefits
Applies clinical judgment to incorporate strategies designed to reduce risk factors
Conducts assessments from various sources, addressing conditions impacting functionality
Uses a holistic approach to assess the need for referrals to clinical resources
Collaborates with supervisors and stakeholders to overcome barriers in meeting goals
Utilizes case management processes in compliance with regulatory and company policies
Utilizes motivational interviewing skills for maximum member engagement
Requirements
Minimum 3+ years clinical practical experience as a Registered Nurse (RN)
Active and unrestricted Michigan RN licensure
Willingness and ability to travel up to 25%-50% of the time in specified counties
Reliable transportation required with mileage reimbursement
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint
Effective computer skills including navigating multiple systems
Confidence working at home/independent thinker
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
2+ years of experience in case management, discharge planning, and/or home health care coordination (preferred)
Behavioral health experience (preferred)
Bilingual in Spanish or Arabic (preferred)
Certified Case Manager (preferred)
Additional national professional certification (CRC, CDMS, CRRN, COHN) (preferred)
