Case Manager, RN – Field

Role Overview

• Develops a proactive plan of care to address identified issues to enhance member outcomes
• Uses clinical tools and information/data review to evaluate member's needs and benefits
• Applies clinical judgment to incorporate strategies designed to reduce risk factors
• Conducts assessments from various sources, addressing conditions impacting functionality
• Uses a holistic approach to assess the need for referrals to clinical resources
• Collaborates with supervisors and stakeholders to overcome barriers in meeting goals
• Utilizes case management processes in compliance with regulatory and company policies
• Utilizes motivational interviewing skills for maximum member engagement

Requirements

• Minimum 3+ years clinical practical experience as a Registered Nurse (RN)
• Active and unrestricted Michigan RN licensure
• Willingness and ability to travel up to 25%-50% of the time in specified counties
• Reliable transportation required with mileage reimbursement
• Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint
• Effective computer skills including navigating multiple systems
• Confidence working at home/independent thinker
• Excellent analytical and problem-solving skills
• Effective communications, organizational, and interpersonal skills
• Ability to work independently
• 2+ years of experience in case management, discharge planning, and/or home health care coordination (preferred)
• Behavioral health experience (preferred)
• Bilingual in Spanish or Arabic (preferred)
• Certified Case Manager (preferred)
• Additional national professional certification (CRC, CDMS, CRRN, COHN) (preferred)
