Role Overview
- Understand and implement person-Centered thinking
- Facilitate the person-Centered planning process
- Coordinate services and health benefits for members
- Consult with members, families and legally responsible people on care needs
- Monitor service delivery for appropriateness of care
- Document care coordination activities and member response in a timely manner
Requirements
- High School Diploma or GED equivalent required
- A minimum of one (1) year of experience working with developmentally or intellectually disabled or behavioral health clients
- Intermediate proficiency level with Microsoft Office, including Outlook, Word and Excel
- Ability to communicate effectively with a diverse group of individuals
- Knowledge of local, state & federal healthcare laws and regulations & all company policies regarding case management practices
- Must have valid driver’s license, vehicle and verifiable insurance
