Manage a caseload of 60–70 youth in the care of DCFS
Complete required assessments to identify short/long-term behavioral and physical health needs
Develop and maintain individualized plans of care with the Interdisciplinary Care Team (ICT)
Conduct contractually required assessments and document findings
Identify member needs and gaps in care (behavioral + physical)
Monitor progress and adjust care plans as needs change
Provide education to members and their families on procedures, healthcare provider instructions, treatment options, referrals, and healthcare benefits
Requirements
Requires a Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing
2 – 4 years of related experience
For Illinois Youth Care plan only: Graduate degree and independently licensed behavioral health clinician (e.g. LCSW, LCPC, PsyD) in Illinois or Bachelor’s Degree and IL RN licensure
Must reside in IL required
