Role Overview
- Evaluate physical and functional status of the patient and caregiver
- Develop a therapy plan of care in collaboration with patients/families
- Provide patient/family education and counseling
- Teach home exercise programs and compensatory techniques
- Document assessments, interventions, and communications
- Communicate pertinent medical and clinical information to the team
- Participate in team reassessments and discharge planning
- Evaluate and assist in obtaining adaptive equipment
- Manage assigned caseload and communicate changes in Plan of Care
Requirements
- Bachelor's or Masters Degree Occupational Therapy
- 1 year Experience in acute care, rehabilitation, or home care
- California Occupational Therapist License (Vendor Managed)
- National Provider BLS
- American Heart Association (Vendor Managed)
- Coursework/Training Training in Treatment of Adult Hemiplegia and Hand Therapy preferred
