Connect members to community services that help meet their social needs.
Address the social determinants of health – such as housing, food access, transportation, utilities, employment, etc.
Serve as the primary liaison, facilitating communication, coordinating joint initiatives, and advocating for resources and support.
Conduct screening to identify barriers (food insecurity, housing instability, etc).
Refer members to community-based organizations using a closed-loop system.
Follow up to ensure referrals were successful and the members’ needs have been met.
Document all interactions in eCare.
Develop and maintain strong relationships with members and Community-Based Organizations to support collaboration.
Collaborate with internal and external teams to provide support to members with their social needs.
Requirements
High school diploma or equivalent required.
At least 2 years of experience in community engagement, partnership management, customer services or a related role within the health care or social services sector.
Demonstrated ability to build and sustain relationships with diverse community organizations.
Strong oral and written communication capabilities, bilingual preferred.
Proficiency in Microsoft Office Suite and familiarity with CRM or partnership management software.
Bachelor's degree in Public Health, Social Work, Health Administration, or a related field preferred.
Experience working specifically with underserved or marginalized communities.
Knowledge of local health care systems, policies, and funding mechanisms.
Project management certification or experience managing complex initiatives.
