Bilingual Nurse Practitioner

Role Overview

• Collaborating with company physicians and other providers to develop complex plans of care
• Providing clinical and non-clinical support to patients
• Providing urgent health care via telephone and tele video modalities
• Developing and implementing clinical plans of care for adult patients facing chronic and complex conditions
• Gathering history and physical exam and diagnostics as needed
• Developing and implementing treatment plans according to patient’s goals of care
• Identifying and closing gaps in care
• Meeting the patient’s and family’s physical and psychosocial needs
• Educating patients and families about medication usage, side effects, illness progression, etc.
• Maintaining patient medical records and documentation
• Participating in continuing education as required by state and certifying body
• Prescribing medication as permitted by state prescribing authority

Requirements

• MS in Nursing
• Active, national NP certification
• Valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in the state of Florida
• Valid, current, active, RN Compact license
• 2+ years of experience in managing complex care cases
• Experience working with Electronic Medical Records (EMR)
• Bilingual English/Spanish
