Role Overview

• Assists in developing, assessing, and coordinating holistic care management activities
• Develops personalized service care plans for long-term care members
• Evaluates member needs and available resources
• Provides educational support to members and their families

Requirements

• Requires a Bachelor's degree
• 2 – 4 years of related experience
• Bilingual preferred
• Ability to assist long-term care members and their families/caregivers
