About this role
Role Overview
- Conduct service coordination functions for a defined caseload of individuals in specialized programs
- Facilitate the Person Centered Planning process
- Conduct assessments and develop comprehensive Person Centered Support Plans
- Engage with members’ support circles and manage individual’s health needs
- Submit utilization/authorization requests
- Report critical incidents to appropriate parties
Requirements
- Requires BA/BS degree
- Minimum of 2 years of experience working with a social work agency
- Bilingual in English/Spanish strongly preferred
- LTSS, case management, social work or hospital discharge planning experience preferred
