Serves as the primary contact for physicians
Communicates with patients and program stakeholders
Educates healthcare providers on program services and resources
Reviews patient charts and ensures completion of required documentation
Takes ownership of special authorization process and ensures timely submissions
Provides ongoing communication and updates to physicians regarding patient status
Adapts program elements to meet the unique needs of healthcare providers
Personalizes support for both in-clinic and remote care
Proactively identifies and addresses areas for quality improvement

Registered Nurse (RN) or related degree (BSc in Pharmacy or other Science degrees)
Knowledge of public or private third-party reimbursement arena
Bilingualism is an asset (requirement for province of Quebec)
Previous Case Manager experience is an asset
Exceptional customer service and interpersonal skills
Ability to manage multiple tasks and priorities
Comprehension of medical terminology
Strong analytical and organizational skills
Ability to engage with physicians and staff as a trusted medical counterpart
Excellent time management skills
Ability to collaborate effectively with program associates
Ability to work autonomously in a remote setting
Ability to communicate effectively in both English and French for Quebec and New Brunswick provinces
Detail and results oriented
Strong computer skills and knowledge of Microsoft Office
Must hold a valid, non-expired, unrestricted driver’s license and possess a car
Must have a private, dedicated home office space that accommodates supplied equipment
